Job Description: To supervise assigned programs effectively and efficiently.  This position report to the program manager and is responsible for supervising assigned operations as business units.  Manages staffing to meet program needs, develops, supports, and supervises assigned leaders.  Oversees training, development, and performance of program staff.  Participates as a member of the organization's leadership and management team.  Develops, promotes, and maintains relationships with referral and other community resources.  Oversees compliance of program and licensing/accreditation functions.
Qualifications:  Minimum qualifications for this position include QMRP status; a 4 year Bachelor's Degree in Social Work, Human Service, Behavioral Science, or related field, a minimum of one year experience working with DD adults and previous supervisory experience or Designated Coordinator status; an Associate's Degree in Social Work, Human Service, Behavioral Science, or related field and a minimum of two years experience and previous supervisory experience.  A valid driver's license and/or demonstrated ability to travel frequently to assigned branches, administrative office, and other locations as needed; excellent organizational, communication, problem solving, attention to detail, personal and professional boundaries, and computer skills; the ability to think creatively and globally, manage multiple priorities, and adapt to change.

Job Description: Reports to assigned supervisor and is responsible for performing individual living and/or employment support services as assigned.  Responsible for following individual program plans as developed by interdisciplinary teams. Communicates participant's program progress to appropriate person(s) in a timely manner.  Completes documentation and required administrative duties as assigned.  Cooperates as a member of the assigned program team.  Functions
as a mandated reporter. Demonstrates knowledge and has the ability to describe program services.  Properly handles participant monies and property as assigned and follows policy and procedures.
Education: HS Diploma or GED
Job Qualifications: Experience working with adults with special needs.  Must have valid MN driver's license and/or demonstrated ability to travel in and between surrounding communities as assigned.

Purpose: This position provides coordination and oversight services to an assigned caseload, including: ensuring quality services are provided, development and documentation of program plans, communication with interdisciplinary teams, administrative reporting, and compliance with licensing and accreditation requirements. 
Job Responsibilities:  Responsible for developing and implementing the individual program plans and ensures optimal programming is provided to participants.  Ensure services are implemented as prescribed in individual program plans, as directed by the individual service plan and within the authorized hours.  Participate in intake meetings, explaining the program and the individual services available.  Develops individualized program plans, progress reviews, and required documentation.  Understands current policies, trends, and practices within the field of services to adults with special needs.  Effectively role models professional behaviors/attitudes to other staff members.  Demonstrates flexibility in responding to situations as participant;/staff needs dictate.
Job Qualifications:  Minimum AA Degree and 2 years experience working in Human Services.  Designated Coordinator status preferred.  Valid driver's license and/or demonstrated ability to travel within the community.  Must demonstrate excellent communication, problem solving, attention to detail, and personal and professional boundaries.
